Enter Inspection report data and submit report

1. Select a segment from the Segment drop-down, if applicable.

2. Select a roadway from the Roadway drop-down, if applicable.

3. Select a structure from the Structure drop-down, if applicable.

4. Select a work type from the Work Type drop-down, if applicable.

Note: The Work Type will automatically fill with the work type you select at the beginning of the Inspection report.

5. Select a contractor from the Contractor drop-down, if applicable.

6. Select a foreman from the Foreman drop-down, if applicable.

7. Verify that the weather has been auto-populated.

8. If the weather impacted the work done, click the Yes button beneath the Weather Impacted text. This will pull up the Impact Notes text box, where you are able to enter the impact the weather had on the work completed.

9. Enter the working duration (hours) in the Working Duration text box in the Time section under Production Duration, if applicable.

10. Enter the inspection duration (hours) in the Time on Site text box in the Time section under Inspection Duration, if applicable.

11. Enter all applicable equipment in the Equipment section.

a. Select the plus button on the right side of the Equipment section. A pop-up will open with entry boxes for applicable information regarding the equipment seen.

b. Enter all applicable data into the appropriate text box or drop-down.

c. Click the Save button. Your equipment information will be shown under the Equipment section.

d. Repeat steps a–c for all equipment.

12. Enter all applicable personnel in the Personnel section.

a. Select the plus button on the right side of the Personnel section. A pop-up will open with entry boxes for applicable information regarding the personnel.

b. Enter all applicable data into the appropriate text box or drop-down.

c. Click the Save button. Your personnel information will be shown under the Personnel section.

d. Repeat steps a–c for all personnel.

13. If a control point (CP) was performed in conjunction with the current Inspection Report, click the Yes button to the right of the CP Performed text.

a. This will cause the CP# from CIMS app text box to appear. Enter the Report ID for the relevant CP in this text box.

14. Enter any notes on testing or sampling performed that are relevant to the current report in the Testing or Sampling Performed This Day section’s text box.

Note: Applicable instructions or comments dependent on the assigned work type will be shown above this text box.

15. Enter any notes on inspection items performed that are relevant to the current report in the Suggested Inspection Items section’s text box.

16. Enter any notes on verbal instructions received or given in the applicable text box in the Verbal Instructions section.

17. Enter any notes on the Maintenance of Traffic in the Associated MOT text box.

18. If there were unresolved issues associated with the current Inspection report, click the Yes button to the right of the Unresolved Issues Associated with this Work text. If the Yes button has been selected, additional selections will appear.

a. Select if the issue was a CDR, NCR, or Punch List item.

b. Select if the issue was New or Pre-existing.

i. If the issue was new, enter your notes on the issue in the New Issue Description text box that appears.

ii. If the issue was pre-existing, enter the CDR, NCR, or Punch List number in the CDR, NCR, or Punch List # text box.

19. If an engineering judgment was exercised, click the Yes button in the Engineering Judgment section to the right of the Exercised text. If the Yes button has been selected, additional selections will appear.

a. Select the engineering judgment item from the Item drop-down. Once this is done, additional information will appear that relates to the engineering judgment item selected.

b. Write any notes associated with the engineering judgment item in the text box below it.

20. Attach any files, images, or videos that are required for your report using the media buttons.

21. Once you have completed entering all data in your report, click the Submit For Review button. This will submit your report for your assigned reviewer to review. Doing this will lock your report so no data can be edited. If the reviewer determines that there are issues or missing data in your report, they will deny the report, which unlocks it and allows you to fix any missing or incorrect data. Once you have completed the fix, you may submit the report for review again. This process will continue until the reviewer approves the report.
